SEMPHN is a leader, facilitator and influencer towards the shared goal of better primary health care. The Access & Referral (A&R) team provides up-to-date access, information and support to SEMPHNfunded mental health and alcohol and other drug services. The Access & Referral team also has extensive knowledge of other local community and health supports. Contact A&R to make a referral, to discuss eligibility or to find services within an area.
