Care finders support vulnerable older people who would not be able to arrange services without with intensive support and do not have a family member or friends who can help. Care finder services are provided at no cost as they are fully funded through Primary Health Networks.

Eligibility Criteria: Do you meet any of the following criteria? > Aged 50 years+ (or aged 45+ for Aboriginal and Torres Strait Islander people). > Need help finding or connecting with services and aged care supports. > Have difficulty communicating because of language or literacy problems. > Are in an unsafe situation or vulnerable if you do not receive services.
