My Aged Care coordinates all assessments and referrals for community services programs across Australia. If you would like to join one of the Social Connections programs, contact My Aged Care on 1800 200 422 or through myagedcare.gov.au. Social connections program includes: group activities, Individual support at home, Telelink program Tech Buddies Pilot Program - help with using technology - only available for Commonwealth Home Support Programme customers only, not receiving a Home Care Package.
